Senior Assessor and Caseload Co-ordinator Band 6, 18hrs per week
The Community Neurological Rehabilitation Team based in Worthing and covering the South East coastal and rural area is seeking to employ an experienced therapist/nurse to join the team. You will be working with clients who have neurological conditions such as Parkinson's disease, Multiple Sclerosis, Motor Neurone Disease and Stroke.
The role will be to assess new clients in the community setting for multi-disciplinary team assessments identifying nursing, therapy and social care needs. You will also oversee the management of existing clients in conjunction with the caseload co-ordinator in order to ensure therapy plans and reviews are carried out. You will work alongside the other therapy and nursing staff in planning a multi-disciplinary approach to care.
Applicant requirements You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.Company
