Floating Support Officer - Mental Health Acute Settings Hospital Discharge

£20,273 – £22,234 per annum plus £1,000 per year essential car users allowance

37 hours per week

The West Sussex Homelessness Prevention partnership service delivers a range of quality services to vulnerable people across West Sussex, offering housing related and accommodation based support. The service contract is due for review at the end of September 2019, therefore this post is currently fixed term but we are hoping the contract will be extended or alternative funding sourced.

Southdown is looking to recruit a Housing Support Officer for a co-located position within Langley Green Hospital. The office base will be the Worthing office but you would, mainly lone-work remotely, meeting clients on the hospital wards. You will accept referrals from ward professionals for patients who are identified as homeless or are at significant risk of becoming homeless and will deliver targeted support in relation to housing and welfare benefits.

To date this co-location has been very successful and the feedback from stakeholders extremely positive.

The key priority for the service is to ensure that patients are discharged to suitable accommodation.

The role involves managing multiple relationships, positively engaging with patients, ward staff, local authorities and voluntary agencies. We are therefore particularly interested in applications from individuals who are skilled communicators, with excellent time management/organisational abilities and with the resourcefulness, resilience and tenacity to work in a busy hospital environment

You will receive a thorough introduction to the wards and will be part of an established and supportive team.

This post requires you to hold a driving licence and have access to a car for work purposes.
