Grade - NHS AfC: Band 4

This is an exciting opportunity to work as an Assistant Practitioner in our Proactive Care Plus/Community Nursing team.

You must be an enthusiastic, motivated individual with good communication and organisational skills who can bring experience and knowledge to the post.

The successful applicant will work independently under the guidance and direction of the senior nursing team to provide efficient and effective evidence-based nursing care to patients in the community. You will be expected to support and deliver a range of clinical activities and support the team in line with patients' individual care and treatment plans.

Applicants must have a foundation degree in Health and Social Care (Adult Nursing), as well as excellent clinical, communication and team working skills.

We offer a comprehensive induction programme and excellent ongoing professional development opportunities.

Your role will include participating in audit, and supporting student nurses on placement with the team.

A willingness to develop new skills and embrace change is expected. You will need the ability to work autonomously and demonstrate initiative with sound clinical knowledge and judgement.

Informal visits prior to interview would be welcomed.
