Nursing Associates - Horsham Community of Practice

Horsham Community of Practice is based in Rose Wing, Horsham Hospital and covers the town of Horsham and the surrounding villages. The team is multi-disciplinary, comprising of health, social, mental health practitioners and Age UK Link Worker.
Horsham COP Team are looking for a dynamic and enthusiastic nursing associate who wishes to work or is already working in the challenging but rewarding setting of the community; Caring for patients in their own home; by providing advice and support to patients to assist with self-management in their health journey including end of life care.
This post is suitable for a qualified nursing associate, with at least 1-2 years of experience, working in either the community or acute setting, where there are transferable skills.
Professional development is available in the post- including leg ulcer and wound care courses; End of Life Care, as part of Sussex Community NHS Foundation Trust commitment to developing the workforce.
In the team you will be working with and be supervising Band 3 Health care assistants; also development of student nurses; and be seen as a role model. Assistant care practitioners are essential to the team and you will be expected to work with the Community staff nurses and Band 6 Case Managers.
Clinical direction is provided by the three Clinical Leads and the team is overseen by the Team Lead. Good wound care knowledge and assessment skills are beneficial, including communication and IT skills.
Car driver is essential.
Staff who work for Sussex Community NHS Foundation Trust and are at risk of redundancy will be given priority consideration for this vacancy.
A Foundation Degree in Health and Social Care is required to apply for this job to work as a Band 4.
