Band 5

Sussex Community NHS Foundation Trust (SCFT) is the largest community health and care provider in Sussex. Our expert teams deliver a wide range of medical, nursing and therapeutic care to over 9,000 people a day, serving a total population of 1.3 million.

We pride ourselves in providing excellent care at the heart of the community. In July 2018, the Care Quality Commission (CQC) gave us an improved 'Good' rating, with 'Outstanding' features. 96% of our patients said they would recommend our services to their friends and family, and we attribute this to the hard work and dedication of each and every one of our teams.

We offer our staff a multitude of benefits; flexible working, free health and wellbeing checks, three tax efficient workplace nurseries rated 'Good' and 'Outstanding' by Ofsted, and a wide range of exciting learning and development opportunities including external courses, conferences, and a Graduate Studentship Scheme.

The Trust is signed up to the disability confident scheme and we guarantee to interview all disabled applicants who meet the minimum criteria for the vacancy.

We currently have a vacancy for a community staff nurse in our Community Nursing Team, based in Crowborough. The team covers both town and rural practice populations across the Ashdown Forest, Crowborough, Wadhurst and Mayfield.

Our team works closely with our linked GP partners, social services, specialist nurses and local hospices to deliver excellent care in the community. We are well supported by a Trust which understands community nursing and delivers adult nursing care to patients across Sussex.

Your responsibilities will be:
  • Visiting patients in their own homes and providing nursing care. This will usually mean working alone, although you will have support from a well-established and experienced team in Crowborough.
  • Carrying out nursing interventions such as wound care, medicine administration, diabetic management, IV drug administration, end of life care and syringe drivers, along with any other nursing interventions provided by the community team.
  • Ensuring that you document all interventions and relevant information on our electronic patient record system (SytemOne).
  • Supporting the caseload managers in ensuring the smooth running of the office and the team.

What we can offer you
  • An environment in which learning is encouraged and supported.
  • Help to develop skills and competencies via training and practice based learning.
  • An effective appraisal system in which you are valued as an individual and helped to plan for the future with the organisation.
  • A perceptorship programme when you join the Trust to help you integrate.
  • Clinical supervision to encourage reflective practice.
  • The chance to become part of a wonderful team of people who look after one another and have a culture of kindness.

You must be able to travel around the Trust in a timely manner, so must have a full driving license.

Informal visits prior to interview are welcomed.

This post may close earlier than the published closing date if sufficient applications are received.

Correspondence relating to this vacancy will be conducted electronically. Please ensure that you check your e:mails on a regular basis. References will be requested electronically also; please ensure you provide correct contact details for referees.
