Are you looking for a new challenge? We are looking for an experienced senior clinical pharmacist who wants to make a difference to patient care as part of the Brighton & Hove Responsive Services team, a multidisciplinary team (MDT) delivering care to patients in order to prevent admissions to hospital. As well as leading the small pharmacy team in Responsive services you will benefit from being part of the Trust's expanding and dynamic medicines management team.

The role: The post of Senior Clinical Pharmacist is a job-share with the current post holder. You will work with all of the members of the MDT in Responsive Services which include: nurses, occupational therapists, physiotherapists, community reablement assistants, advanced practitioners, social workers and mental health liaison nurses. The post also includes providing medication review and MDT input to the Osteoporosis and Falls Prevention service. Including running a postural hypotension clinic. You will conduct in depth medication reviews with patients in their homes and make recommendations to GPs to improve patient outcomes focusing on quality and safety. You will establish and maintain effective working relationships and networks, liaising with GPs, community pharmacies, social services as required to implement medicines optimisation.

The person: We are looking to appoint highly enthusiastic, committed and self-motivated individual able to work in a fast paced environment in a dedicated team. You will need to be able to prioritise work and understand the importance of key performance indicators to demonstrate the difference made to patient care and patient outcomes. The post holder requires a carefully managed, constructive and collaborative approach to ensure that this translates into improved patient outcomes. You will need significant post qualification experience relevant to the post. A Clinical Diploma or robust relevant CPD is essential and applicants are welcomed from all branches of pharmacy practice.

You will be expected to travel across the geographical area covered by Brighton & Hove CCG. You will also be expected to periodically travel to other areas covered by Sussex Community Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).
