An exciting opportunity has arisen to join our busy and essential Capacity and Resilience Team as Resilience Practitioner. We are a small team which plays a vital role in the flow of patients through the health and social care system from the acute trust through to community beds and into our community services. This key role involves providing in-reach into the acute and community hospitals in order to review the referrals and ensure patients are in the best place to support their recovery and ensure safe discharge of patients. In the acute the referrals will come from the wards but also from the Emergency Floor, so as well as discharge planning, experience in admission avoidance will also be beneficial.

There are also a number of innovative work streams including the trusted assessment process and discharge to assess which the post holder will also have the opportunity to be involved in. This role has a positive direct impact on delayed transfers of care and length of stays in both acute and community hospitals. This is an excellent opportunity for any AHP or nurse to put into practice their knowledge and experience in assessment and management of complex patients, discharge planning, rehabilitation, and intermediate and continuing care as well as to develop within the role and organisation.

We are looking for someone to support the discharge processes and escalate any delays within the community beds through effective liaison with our partner organisations and other stakeholders to ensure a seamless transfer of care with minimal delays. You will be covering either Worthing or St Richard's acute hospitals and some of the 5 Sussex Community Foundation NHS Trust West area hospitals so a car driver is essential. Flexibility to cover for colleagues at other sites across SCFT and acute hospitals will also be essential.

You will be a confident facilitator and skilled communicator with a flexible and organised approach. The successful individual will contribute positively to the development and growth of the team and to the quality of the service received by our patients. In return, we offer you professional support and mentorship and we actively encourage innovative practice within a multidisciplinary team. We welcome applications from Physiotherapists, Occupational therapists, nurses, social workers and anyone who has proven experience in discharge planning.
