Are you looking for a new challenge? Are you an existing Band 6 Nurse or a Senior Band 5 looking for progression, and to build on your experience in the community?

An exciting opportunity has arisen for a 12 month fixed term/secondment as a Community Sister in a multi professional team within the community environment. This includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The role is that of a caseload holder, providing clinical case management for patients with complexities of health needs who are at risk of deterioration which may result in declining quality of life, avoidable hospital admissions, or an unnecessary length of stay.

The role includes using your clinical skills for assessment, diagnosis, treatment and/or appropriate referral in collaboration with other agencies to ensure care coordination, and includes being the named key link for individual GP practice(s). As an experienced clinician you will provide leadership and mentorship to other more junior members of the team, and have delegated responsibility for a team of nursing professionals.

You will be working within a multidisciplinary team. The role is based in an integrated hub of experienced and supportive nurses and has direct access to the specialist teams within the hub.
