Senior Clinical Pharmacist - (Medicines Optimisation for Care Homes - Learning Disabilities Lead)

Are you looking for a new challenge? We are looking for an experienced clinical pharmacist who wants to make a difference to patient care by practicing and delivering medicines optimisation to care homes for people with a learning disability and help support medicines optimisation for older people residing in care homes.

This is a new post to join the established Medicines Optimisation for Care Homes Service team which forms part of Sussex Community NHS Foundation Trust; an expanding, dynamic and proactive Medicines Management Team. The post holder will be responsible for developing and providing specialist prescribing and medicines optimisation support to Learning Disabilitiy Care Homes.

The team consists of pharmacists and pharmacy technicians delivering the Medicines Optimisation for Care Homes service, working in geographical patches e.g. Eastbourne, Bexhill-on-Sea and Hastings, predominantly in care homes and link GP surgeries.

The role: As a Medicines Optimisation for Care Homes Pharmacist you will be delivering medicines and health reviews with residents. You will review prescribing and make recommendations to GPs to improve patient outcomes focusing on quality and safety. You will establish and maintain effective working relationships and networks, liaising with GPs, community pharmacies, care home managers, social services and community specialist health services e.g. Sussex Partnership foundation Trust, and other stakeholders as required to implement medicines optimisation.

The person: We are looking to appoint a highly enthusiastic, committed and self-motivated clinical pharmacist with an understanding of the medicines optimisation issues within learning disabilities and the care home setting. You will need to be able to prioritise work and understand the importance of key performance indicators to demonstrate the difference made to patient care and patient outcomes. You will need significant post qualification experience relevant to the post and have a proven track record of successful implementation of medicines optimisation projects. A Clinical Diploma or robust relevant CPD is essential and applicants are welcomed from all branches of pharmacy practice. Knowledge of prescribing in the frail elderly population and learning disabilities in care homes is desirable.

The successful candidate may be enrolled on the NHS England Medicines Optimisation in Care Homes (MOCH) training programme (18 months) which includes an independent prescribing qualification.

You will be expected to travel across the geographical area covered by Eastbourne, Hailsham, Seaford, Hastings and Rother CCGs. You will also be expected to periodically travel to other areas covered by Sussex Community Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).
