Senior Pharmacy Technician- (Medicines Optimisation for Care Homes)

Are you looking for a new challenge? We have a very exciting opportunity for a dynamic individual to join our Medicines Optimisation team.

We are looking for an experienced Pharmacy Technician to join the Medicines Optimisation for Care Homes team which forms part of Sussex Community NHS Foundation Trust; an expanding, dynamic and proactive Medicines Management team.

The post holder will strive to make a difference to patient care by improving medicines management with older people and people with learning disabilities living in care homes.

The team consists of pharmacists and pharmacy technicians delivering the Medicines Optimisation for Care Homes service, covering geographical patches within Eastbourne, Hailsham and Seaford CCG and Hastings and Rother CCG working predominantly in care homes and link GP surgeries.

The role: You will be working in partnership with care homes to review medicines management systems and processes producing agreed action plans with recommendations in line with local and national guidance. The role requires close liaison with GP surgeries and community pharmacies to improve medicines safety, efficiency and reduce waste. You will support and develop the Medicines Optimisation Team objectives by contributing to reports, establishing trends in practice, writing and implementing best practice guidance.

The person: We are looking to appoint a highly enthusiastic, committed and self-motivated individual able to work in a fast pace environment in a dedicated team. You will need to be able to manage your time effectively, prioritise your own workload and understand the importance of key performance indicators to demonstrate the difference by your interventions. Registration with GPhC and an accredited Medicines Management qualification and /or extensive experience is essential. Applicants are welcomed from all branches of pharmacy practice with a special interest in frailty and learning disability with supporting CPD.

The successful candidate may be enrolled on the NHS England Medicines Optimisation in Care Homes (MOCH) training programme (18 months) and will work alongside colleagues undertaking similar roles to improve medicines management for residents in care homes.

You will be expected to travel across the geographical area covered by Eastbourne, Hailsham, & Seaford or Hastings & Rother CCGs. You will also be expected to periodically travel to other areas covered by Sussex Community NHS Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).
