We have a vacancy for a Band 6 Community Nurse to join our integrated multidisciplinary team in Responsive Services, based in Haywards Heath. The service runs between 08:00 and 20:00 over seven days a week, and a car driver is essential.

This post would suit an enthusiastic and innovative individual who is prepared to meet the changing needs of the population we serve and provide high quality, flexible care where it is needed.

Responsive Services includes admission avoidance - a rapid response service in the community aimed at preventing unnecessary hospital admissions and helping people to stay in their own homes - and supported discharge/rehabilitation, providing short term rehabilitation for patients in the community. The team assess patients at home, identify patient needs, set care plans, and provide regular reviews, adjustments, and discharge planning. Responsive Services work alongside the acute services and local community hospitals to facilitate discharge, liaising with Communities of Practice, GP surgeries, social workers, and local charity organisations.

The team uses electronic patient records and individual laptops for mobile working to aid patient-facing time, so general IT knowledge is essential.
