The Horizon Unit

Horsham, West Sussex.

We currently have a vacancy for a

Band 6 Occupational Therapist - 30 hours (ward based)

The Horizon Unit is a 38 bedded unit in Horsham, West Sussex with close links to other community and in-patient services. We pride ourselves in caring for adult patients of all ages with a variety of complex medical conditions and those who have post-surgical rehabilitation needs.

Our Multi-disciplinary team includes a Ward Doctor, Advanced Nurse Practitioner, Occupational Therapists, Physiotherapists and an onsite Social Worker. To help our patients experience a high standard of care we also have input from Specialist Nurses, a Consultant from our Local Acute Trust and community teams to support discharges from hospital.

Horizon is a friendly stand-alone unit. The care and therapy our patients receive is delivered through careful patient centred assessment with involvement from the patient and their families. We are very lucky to have gardens which been adapted for use by patients; a therapy gym and a day/dining room enabling us to provide an activity timetable to maximise patient recovery and ensure their admission is a positive experience for both them and their relatives. The feedback we have received from patients is that receiving care in this environment has been invaluable to them on their journey after an acute event.

To be successful in the post you must demonstrate a keen interest in care of the older person, the delivery of quality patient rehabilitation and discharge planning within a community hospital environment. You must be able to demonstrate actions that will impact on timely patient assessment, goal setting and treatment and discharge planning. We are looking to expand our current group activities and this post would be pivotal in implementing this.

Our recent Care Quality Commission inspection rated our community in-patient services as Outstanding for caring and our overall Trust rating as Good. If you are passionate about making a difference and want to contribute to an organisation which is continuously striving to improve then we would love to hear from you.

In return we can offer you: • Flexible working patterns • Supportive learning environment • Dedicated career pathways and great opportunities for continuing professional development including mentorship and physical assessment modules • Commitment to and support for your health and wellbeing • NHS Pension Scheme • NHS discount and salary sacrifice schemes, including childcare vouchers

-The surrounding area has good transport links to London and Brighton.

-We have onsite parking and are 15 minutes' walk away from the train station and local town.

If you are committed to providing care in a compassionate and safe environment then working at the Horizon Unit will be an opportunity for you to use your existing skills, gain new ones and be part of a great team. Apply now!
