We are looking for an enthusiastic, highly motivated and well-organised Band 5 nurse or therapist to work in the Referral Management Hub in Brighton. The Hub supports all the referrals to Brighton and Hove Responsive Services and the rehabilitation beds in Brighton.

The role involves the triage and coordination of referrals that support early hospital discharges and avoidable hospital admission. Referrals come from hospitals, GPs, paramedics and other agencies.

The post holder will be required to work as part of a team, but also independently, prioritising referrals using clinical decision-making skills, and seeking support from senior staff to manage complex referrals as required.

The post holder will ideally have an understanding of the services available across our Trust, and be able to effectively communicate with a wide range of people including clinicians, therapists, social care workers, managers, staff, and patients using the services.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
