Diabetes Care for You are looking for a Team Administrator to work in the referral management hub, processing incoming referrals and managing patient bookings to ensure patients are seen in a timely manner in the location of their choosing.

This is a fixed term post for 12 months to cover maternity leave. There is 1 full time post (37.5 hours) and another post for 27 hours per week.

The post holder must have good organisational and communication skills, and be able to work under their own initiative and follow directions. They must also be engaged in service development for both the administrative and clinical processes to maximise efficiency and ensure a good patient experience.

The successful applicant must be able to act as a gate keeper between patients and clinicians, and be able to appropriately glean pertinent information to ensure patients are signposted to the correct support/service for their query. Prioritisation and negotiation skills are therefore key to this role.

The community diabetes service is a multidisciplinary team which includes Consultants, Diabetes Specialist Nurses, Dietitians, Podiatrists and Psychotherapists. Its aim is to improve health outcomes for people at risk of diabetes or who have a diagnosis of diabetes, and to meet the needs of the growing diabetes population across Brighton and Hove, and High Weald, Lewes and the Havens, both by providing interventions to people with diabetes and by increasing the level of skills and knowledge within primary care.

The goal of the service is to enable patients to access education, information, advice and support to help them to make informed choices about how to manage their condition, so that they can stay as well as possible for as long as possible, and reduce their risk of diabetes-related complications. Joint care planning and goal setting will be a core focus of the new delivery model, with patients in control and empowered to self-manage their health.
