Are you looking to develop your skills and knowledge by becoming a senior clinician in the Referral Management Hub?

The Referral Management Hub is part of Responsive Services, a community service which delivers the 'right care at the right time in the right place'. Our Responsive Services teams play a key role in supporting patients' discharge from hospital and community beds, preventing avoidable admission to hospital, and supporting patients to stay at home.

An exciting opportunity has arisen to join the Referral Management Hub as a Senior Referral Management and Triage Coordinator, playing a fundamental role in managing and supporting referrals into Responsive Services.

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
