Band 6 Occupational Therapist - up to 37.5 hours available working across working across high weld lewes and havens site (not a specific site as base) Weekend working will be expected.

This post works closely alongside our physiotherapists, therapy practitioner and rehabilitation support workers, together with the full MDT. Our Multi-disciplinary team also includes a Ward Doctor, Nurses, Social Workers, as well as input from Specialist Nurses, a Consultant and community teams to support discharges from hospital.

Our units offer inpatient rehabilitation to the local population. We pride ourselves in caring for adult patients of all ages with a variety of complex medical conditions, some end of life patients and those who have post-surgical rehabilitation needs in a friendly and stand-alone unit. The care and therapy our patients receive is delivered through careful patient centred assessment with involvement from the patient and their families. We are very lucky to have a therapy gym/dining room enabling us to provide an activity timetable including a breakfast group to maximise patient recovery and ensure their admission is a positive experience for both them and their relatives. The feedback we have received from patients is that receiving care in this environment has been invaluable to them on their journey after an acute event.

To be successful in the post you must demonstrate a keen interest in care of the older person, the delivery of quality patient rehabilitation and discharge planning within a community hospital environment. You must show high levels of communication with patients, their carers and the MDT. You must be able to demonstrate actions that will impact on timely patient assessment, goal setting and treatment and discharge planning. The post will be based at Uckfield and Crowborough, but may require you to work across all 3 sites as the service needs. We are looking to expand our current group activities and this post would be pivotal in implementing this.
