We are seeking to recruit a Diabetes Specialist Nurse to work within Diabetes Care for You covering Brighton, Hove, Lewes, High Weald and the Havens in East Sussex.

This is an exciting opportunity to work within a multidisciplinary team of consultants, nurses, dietitians, podiatrists, psychologists and support staff, and contribute to the development of this innovative service.

The Diabetes Specialist Nurse will work in partnership with patients with type 1 or 2 diabetes in a community setting, aiming to improve outcomes for patients with diabetes. This includes the development and delivery of clinic-based care, home visits, and patient education programmes for diabetes management.

The Diabetes Specialist Nurse will also establish and maintain effective working relationships with health professionals in both primary and secondary care to ensure the provision of high quality specialist diabetes care to patients and carers at all stages of the care pathway.

This role offers an excellent opportunity to develop clinical and service development skills, as well as experience of working within a multidisciplinary team, integrating physical and psychological support.
