Opportunities have arisen within the Responsive Services team in Brighton and Hove for highly-skilled, motivated and enthusiastic individuals to join our team. Based at Brighton General Hospital, we deliver services across the city of Brighton and Hove within patients' normal places of residence.

As a nurse you will Join our multidisciplinary team which consists of an Advance Nurse Practitioner, Nurses, Occupational Therapists, Physiotherapists, Registered Mental Health Nurses, Social Workers, Pharmacists, Community Reablement Assistants and Community Care Assistants. Working collaboratively with our acute Trust and other community services, including third sector organisations, we aim to improve the patient journey and pathways of care and deliver short term intervention, care and support in the community.

Responsive Services delivers the 'right care at the right time in the right place', playing a key role in supporting patients' discharge from hospital and community beds and supporting them to stay at home, preventing avoidable admission to hospital.

SCFT offers excellent learning and development opportunities for all staff, regular supervision and appraisal, and a commitment to supporting employees in developing their careers within the organisation.

You will need a UK driving licence and use of a car for business purposes.
