We are looking for dynamic and enthusiastic Community Sisters to join our Arun Primary Care Network (PCN) team based out of Zachary Merton Hospital in Rustington. This an ideal opportunity for an experienced Band 5 nurse looking to develop their clinical and operational skills.

The emerging PCNs are groups of practices working together in collaboration with community and other health and care services. PCNs enable greater provision of proactive, personalised, coordinated and integrated health and social care.

Our Community Sisters have overall responsibility for designated caseloads, and manage care delivery by the integrated team. The post holders will attend health and social care meetings to discuss complex patients at risk of admission, and devise contingency plans accordingly. They will also be responsible for staff management, including personal development reviews, supervision and staff development. Clinical support will be provided by the Clinical Leads.
