SCFT delivers community health services for people living across East and West Sussex. Our coastal West Sussex community services are expanding, and an opportunity has arisen for Band 3 Healthcare Assistants to join our Responsive Services teams in Worthing and Chichester.

Our Responsive Services teams provide short term community healthcare services, with a key focus on facilitating discharges and reducing admissions to hospital. They work closely with medical colleagues, including GPs and community elderly care consultants, providing a joint approach to care delivery. They provide expert interventions and advice to patients and the local community on managing illnesses, and exacerbations which could result in deterioration of health and wellbeing. The teams treat people in the community, so being a car driver and owner is essential.

The successful candidates will work in a wide range of community settings, supported by Registered Nurses and Therapists, and will need to be able to work autonomously as well as part of a team. They will be required to work shifts covering 8am-4pm or 12pm-8pm, on a rota which may include working weekends.

We're looking for people who can demonstrate excellent communication skills, and an understanding of how to help people manage their health conditions in community settings. Previous experience or knowledge and practical skills such as skin and wound care, specimen collection, and taking observations are a bonus, but training and professional development will be provided.
