We are looking for a motivated and experienced professional with the relevant knowledge and commitment to join our team as a System Capacity and Flow Manager and provide excellent clinical leadership.

This role is fundamental to managing the Patient Flow team that works within the Discharge Hub based at the Royal Sussex County Hospital.

As an expert in discharge and patient flow, you will have an integral role within the Discharge Hub, supporting with the coordination of hospital discharge into community beds.

The role involves managing and coordinating a wide range of complex and competing priorities in regards to providing safe and effective discharge planning, and you will be working closely with adult social care and other external partners.

The role will also require you to deputise for the Senior Hospital Discharge Hub Coordinator as required, which will include leading on touchpoint calls and chairing MADE events, so excellent communication skills and knowledge of national patient flow drivers are essential for the role.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
