An exciting opportunity has arisen for an experienced healthcare professional to play a key role in developing the East Grinstead Primary Care Network (PCN) Community team.

PCN Community teams are multidisciplinary teams combining community nursing and proactive care. PCNs are the core element of a wider system responsible for coordinating care delivery to patients on the caseload. Care is organised around individuals' identified needs through collaborative working by service providers.

The Team Lead provides operational day-to-day management and leadership of the team, ensuring service outcomes and performance indicators are met. The post holder will work in partnership other health and social care providers and be the identified link for a specific GP practice population. The post holder will also be responsible for staff management, clinical governance, driving innovative change, and ensuring a high quality service is delivered to our patients.

The post holder will be an experienced healthcare professional with relevant postgraduate qualifications and a management qualification, or be able to work towards these.
