Do you want to work autonomously, whilst being part of a supportive team, and use your clinical skills and judgement to assist in acute community patient care?

If so, come and join Responsive Services in Worthing; we are a friendly, successful and expanding team, and you can enjoy being by the sea and the beautiful South Downs national park.

In Responsive Services, we work hard to avoid unnecessary hospital admissions, and also support patients through the critical first few days after they leave hospital.

Our Responsive Services are made up of multidisciplinary teams providing a seven day service.

We deliver:
  • A rapid response to people in the community who are experiencing acute episodes of ill-health
  • A supported discharge service to facilitate timely hospital discharges
  • Short term support for people who require assistance with activities of daily living
  • A rehabilitation service including a range of therapies and nursing

The team works closely with other community teams within SCFT, GPs, South East Coast Ambulance Service (SECAmb) Paramedic Practitioners, Advanced Nurse Practitioners (ANPs) and Clinical Nurse Specialists, other health and social care professionals across different providers. There is also progress towards working more collaboratively with voluntary and third sector organisations, and carer support services.

The post holders will need to have experience of working within a variety of clinical settings, including acute and community roles.

Car drivers/owners are essential due to the geographic area covered by the team.

As a member of the team you will have the opportunity to:

  • Develop specialist areas of practice, relevant to the team
  • Participate and assist in the implementation of the Calderdale Framework

  • Develop leadership skills through formal and supported training
  • Develop skills beyond the usual scope of practice of your profession
  • Assist in shaping future clinical practice of the nursing team
