Job overview

We have an amazing opportunity for a Registered Professional to join our System Capacity and Flow Team as an In-Reach Practitioner based out of East Surrey Hospital.

Main duties of the job

The In-Reach Practitioner will support the System Capacity and Flow
Manager in managing patient flow from acute providers to and across our Intermediate Care Units. As part of the team, we strive for all bed capacity across the Trust to be used whilst ensuring patients experience safe, high quality care throughout their patient journey.

The In-Reach Practitioner will work with system partners and colleagues to expedite safe transfers and provide safe alternative pathways for patients. This involves working collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised and patient flow is maximised.

The post holder will need to hold a full UK driving licence and have access to a car.

Qualifications and/or Professional Registration
Essential criteria
  • Current NMC registration or other Health Care Professional quantification
  • Degree or equivalent experience
  • Evidence of continuous professional development
  • Relevant postgraduate level qualification/Level M study (Study at Masters level) or equivalent experience


Desirable criteria
  • General management qualification


Experience
Essential criteria
  • Experience of operational management (i.e. caseload management)
  • Experience of partnership working with a wide range of stakeholders and partner organisations
  • Experience of working with multi-disciplinary teams


Desirable criteria
  • Experience of service developments
  • Experience of change programmes


Skills and Knowledge
Essential criteria
  • Ability to engage and work collaboratively with a range of stakeholders
  • Ability to communicate highly complex information to a range of stakeholders and colleagues
  • Ability to interpret data and information from a range of sources and make decisions promptly and decisively to support effective service delivery
