Community Staff Nurse - 37.5 per week

An exciting opportunity has arisen to work in our Community Nursing teams in Brighton & Hove. We are proud of the quality holistic care we provide to the population of Brighton and Hove and focus on self-management with patients using a proactive care approach. The team work closely with a multidisciplinary network of health, social, and third sector staff including Specialist Nurses, hospitals, hospices and GP surgeries.

Main duties of the job

This post is ideal for somebody who would like the opportunity to experience community work or develop their interests in the community setting further. The role incorporates the day to day management of patients within the community under the supervision of the Community Sisters and Charge Nurses and Clinical Lead, involving a variety of skills including wound care, continence management, palliative care, health promotion, venepuncture and more. Training will be available and candidates will be supported with an induction and a personal development plan. Full support will be given to those without community experience to gain the necessary skills required. We would also welcome newly qualified nurses who will be fully supported with preceptorship.

We are a supportive and forward-thinking team with clinical direction provided by a Clinical Lead, overseen by the Operational Team Lead and Clinical Service Manager for Brighton and Hove.

You must be able to work autonomously as well as part of a team. The team use SystmOne mobile working. You must hold a full driving licence and have the use of a car for work purposes.

Detailed job description and main responsibilities
Job summary

a) To deliver multi-professional high quality care in the community, working with patients towards rehabilitation, self-management and independence
throughout the continuum of care.
b) To participate in a team of colleagues supporting evidence based care,
facilitating staff learning and development.

Communication and working relationships

a) Working in partnership, the post-holder communicates effectively with patients and their carers, the multi-disciplinary team and other key stakeholders on a range of complex health care matters, issues, ideas and situations.
b) Acts as an information resource for patients, multi-disciplinary colleagues and other stakeholders. Responsible for signposting and referring individuals, families, carers and communities to the most appropriate agencies/services to support their needs.
c) The post-holder may experience challenge in gaining concordance with the patient, families, or carers for any treatment plan. They may also experience resistance from other colleagues to an agreed action, or treatment option. Strong negotiating and influencing skills may be required.

3. Key responsibilities

a) Holistically assess, plan, implement and evaluate patient care whilst working
in an integrated manner with the multi-disciplinary team.
b) Empower patients to work as partners in their care respecting individual
choice and difference and encouraging self-care where able. Supporting
patients in a programme of self-care/enablement.

Main tasks

a) Holistic assessment and care planning.
b) Individualised care and the promotion of self-care.
c) Safe and effective review and referral to the appropriate member of the
multidisciplinary team.
d) Effective delegation and prioritisation of workload.
e) Professional development as required to maintain area of practice, and as
negotiated in supervision.
f) An awareness of health, safety and security risks.
g) Engagement in service provision and change.

Registration
Essential criteria
  • Registered nursing Part 1(Current registration with Nursing and Midwifery Council)


Desirable criteria
  • 1 year post reg
  • Experience of community nursing


Knowledge
Essential criteria
  • Good communication and negotiation skills
  • Good numeracy and literacy skills
  • Demonstrates competancy in a range of Clinical Skills
  • Demonstrates the application of research based practice
  • Knowledge of current local policies
  • Understanding of the role
  • Promote independance and choice in undertaking patient care
  • IT literate or willing to achieve within an agreed period
  • Knoweldge of clinical goverance, clinical audit and clinical supervision and professional codes of conduct
