Job overview

We are looking for an experienced clinical pharmacist to lead on medicines optimisation for care homes (MOCH) services in Chichester and Bognor Regis regions. Based with community multidisciplinary teams at either Bicentennial House, Chichester or Bognor War Memorial Hospital you will work in care homes across the geography of the local Primary Care Network regions.

As a Senior Clinical Pharmacist you will be the medicines optimisation lead for the local multidisciplinary care home teams in your area. You will be liaising with GPs, Enhanced Health in Care Homes matrons, community pharmacies and others involved in patient care as required to implement medicines optimisation. You will join an existing team of specialist pharmacy staff who will support you to settle into your new role and provide ongoing peer support.

You will need excellent computer skills to work with a range of clinical systems including GP held patient records (EMIS and SystmOne).

You will need your own transport to travel from your home or Trust office base to visit care homes and must meet national covid19 vaccination/testing requirements relating to working in care homes. You will be expected to periodically travel to other areas covered by Sussex Community Foundation Trust.

Main duties of the job

Carrying out clinical medication reviews of care home resident's medication to ensure their medicines are optimised for increased effectiveness, reduced cost and/or wastage and working in partnership with patients to achieve best health outcomes.

Prescribe and/or negotiate changes to prescribing with GPs to promote high quality, evidence based and safe prescribing.

Resolving medicines queries relating to individual residents and supporting safe and appropriate medicines management within care homes, including to minimise the level of medicines wastage.

Proactively developing good working relationships with stakeholders and partners e.g. care homes managers and staff, GPs/primary care clinicians and practice managers, Enhanced Health in Care Home matrons, community health and social care teams

This is a new post to join the established Medicines Optimisation for Care Homes (MOCH) service, leading on medicines optimisation for care homes in Chichester and Bognor Regis and supporting your colleagues to deliver specialist MOCH services across the Coastal West Sussex geography.

Looking to the future, The Coastal West Sussex Medicines Optimisation for Care Homes service is part of an expanding, dynamic and proactive Medicines Management team with roles across East and West Sussex including in-patient, care home, advanced and specialist pharmacy roles, so there will be several opportunities to further develop your career in the future.

The SCFT MOCH service in East Sussex were a PrescQIPP Award winner 2019 and HSJ Awards Finalist 2018. Whilst the Proactive Care pharmacy Service (MO in own homes) was shortlisted for an HSJ Award in 2015.

Detailed job description and main responsibilities
We are looking to appoint a highly enthusiastic, committed and self-motivated clinical pharmacist who ideally has an understanding of medicines optimisation. You will have excellent communication and time management skills. You will need to be resourceful and able to make complex clinical decisions, prioritise tasks and clinical issues and make effective patient centred interventions to improve care and outcomes for care home residents.

As a senior clinical pharmacist you will be delivering structured medication reviews (SMR) with care home residents, carers and nurses, and next of kin. You will review prescribing and make recommendations to GPs to improve patient outcomes focusing on quality and safety. You will establish and maintain effective working relationships and networks liaising with GPs, community pharmacies, care home managers, social services and other stakeholders as required.

You will have excellent computer skills to use a range of clinical systems to inform and document your reviews such as GP held patient records (EMIS/SystmOne) and remote technology for video and telephone conferencing.

Based with the community multidisciplinary teams at SCFT sites (Primary Care Network community teams) in Bognor Regis or Chichester the successful candidate will travel to care homes to provide medication reviews across a geographic region. You will also be expected to periodically travel to other areas covered by Sussex Community Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).

Working hours are Monday to Friday, no weekends or on call. Flexible working may be considered.

Person specification

qualifications
Essential criteria
  • Master degree MPharm
  • Registered pharmacist (1 year competency assessed and examined) professional registration with the General Pharmaceutical Council (GPhC) UK.
  • Diploma in Clinical Pharmacy or equivalent


Desirable criteria
  • MSc in Clinical Pharmacy or equivalent
  • Train the trainer course or similar
  • Non Medical Prescriber


Experience
Essential criteria
  • Extensive post-registration experience with a significant relevant experience as a clinical pharmacist
  • Delivering training to healthcare professionals
  • Clinical audits. Full audit cycle. Plan, organise the collection of data, analyse data, findings, recommendations and audit report writing. Follow-ups
  • Experience of writing policies, procedures and protocols
  • Ability to demonstrate knowledge of working in multi-disciplinary team
  • Knowledge of primary/secondary care interface issues


Desirable criteria
  • Working with a community health services setting
  • Previous experience of working as a clinical pharmacist in care home and a good understanding of this care setting
  • Previous experience of working in GP practices and familiar with some of the GP computer systems used.
