An exciting opportunity has arisen for a highly motivated and experienced Advanced Nurse Practitioner to join our expanding and innovative Community Responsive Service.

Main duties of the job

We are seeking a dedicated practitioner with Advanced Physical Assessment and Non-Medical Prescriber (NMP) qualifications who is committed about working within the field of community urgent care.

The post holder will work as part of the team in the assessment, diagnosis and treatment of patients who are referred to Responsive Services with a range of care health needs, autonomously managing the episode of care within their scope of practice and experience, and liaising with primary care, acute and community colleagues as appropriate.

In return you will be supported by the Urgent Care Service, receive clinical support from appropriate clinical professionals, managerial supervision from the team manager, and an opportunity to expand your already existing skills.

IMPORTANT: The role will involve regular travel across Brighton and Hove. Please indicate on your application how you can meet this travel requirement.

Detailed job description and main responsibilities
Job summary
  1. The post holder will work as part of the team in the assessment, diagnosis and treatment of patients who are referred to Responsive Services with a range of care health needs, autonomously managing the episode of care within their scope of practice and experience, liaising with primary care, acute and community colleagues as appropriate. Utilising the skills and competencies of an advanced nurse such as prescribing
  2. In addition the post holder will be expected to utilise the additional skills and competencies of an Advanced Practitioner by independently prescribing for patients where required and clinically relevant
  3. To provide advanced clinical nursing knowledge, expertise, leadership and supervision to all relevant staff
  4. To develop links and relationships across the acute Trust and SCFT to ensure safe and clinically effective care for patients.
  5. To provide clinical supervision, support and individual development plans for Responsive Service Practitioners. Be responsible for arranging suitable training, and developing Practitioner skills providing mentorship, teaching and promoting a high level of clinical skill within the team.
  6. Hold responsibility for the development and delivery of a consistently High quality service; promoting the team as a positive place for staff experience and development
  7. Demonstrate a comprehensive knowledge base and promote excellence in nursing practice in a dynamic healthcare environment.
  8. Instigate diagnostic tests/investigations; interpreting and acting on results

Person specification

Qualifications
Essential criteria
  • RGN first level nurse or paramedic
  • Post graduate level degree or equivalent
  • Advanced Physical Assessment course
  • Masters Module/accredited physical assessment course
  • Non Medical Prescriber
  • Advanced Nurse/ Paramedic Practitioner
  • Masters qualification or evidence of study at masters level in a related subject
  • Mentoring Course or equivalent


Experience
Essential criteria
  • Advanced physical assessment and history taking
  • ECG and interpretation
  • Independent or supplementary prescribing
  • medication review and administration
  • Working to and contributing to the creation of patient group directions
  • Experience of using clinical software programs eg System one ,PAS, PACS
  • Safeguarding vulnerable children and adults
  • Ability to empower patients by appropriate health teaching and advice
  • Knowledge of Health and Safety within clinical practice and associated areas
  • Knowledge of local and national policies relevant to clinical area.
  • Experience of clinical audit
  • Knowledge of clinical quality standards, i.e. IC, CQC and evidence of implementing and monitoring these in practice
  • Evidence of extended clinical skills relevant to level of practice such as suturing, plastering
  • Experience with the development of protocols, guidelines and policies

  • Evidence of ability to identify training needs of self and team to provide relevant and appropriate experience to all learners
  • Evidence of one to one mentoring skills
  • Evidence of teaching patients and carers
  • Able to design and deliver individual and group education and training packages
  • Evidence of advanced communication skills, able to understand blocks to communication and ways of developing strategies to overcome these.
  • Assertive and able to negotiate in complex situations.
  • IT literate or willing to achieve within an agreed period.
  • Able to maintain a calm manner in fast changing and emergency situations
  • Ability to work in flexible way and respond positively to change
  • Ability to work autonomously and make own decisions
  • Ability to travel between sites, car owner/driver with access to a vehicle for work purposes
  • Demonstrate a high level of understanding of self and able to identify personal limitation and shows openness to address them

  • Evidence of ability to direct and co-ordinate patient assessment and treatment working autonomously and collaboratively
  • Able to analyse complex situations and problem solve as necessary.
