Job overview

A new opportunity has arisen for a new band 6 nurse or paramedic to join us at responsive services in Horsham. We are a friendly multi-disciplinary team that provides an admission avoidance and early supported discharge service for the local population.

We are excited to be developing a 2- hour crisis response to further prevent unnecessary admissions and support peoples wishes to remain at home at the end of life.

The service operates between 08.00hrs and 0.8.00hrs 7 days per week across shifts.

You will be enthusiastic, able to work autonomously using a wide range of practical nursing skills and have enhanced clinical reasoning. You will have excellent leadership skills, ability to react according to priority and remain calm under pressure. Main duties of the job

This is an exciting opportunity to enhance and develop a wide range of skills including physical assessment, advanced care planning and clinical decision-making in a community setting. We are a rehabilitation and reablement team, focusing on supporting patients to meet their personal goals.

The purpose of the role:
  • To provide a high-quality standard of care to patients who have a wide range of physical needs affecting their ability to maintain a level of independence.
  • Enable patients to manage own health condition and acute exacerbations.
  • Be able to balance risk to support admission avoidance.
  • To provide a holistic and clinical assessment for patients with complex needs, and implement patient centered care working in partnership with the MDT .
  • Provide expert clinical skills in wound care, venepuncture, catheterization and end of life care to support admission avoidance.
  • To provide advice and support to patients, families, carers and voluntary agencies regarding the patient's medical condition ensuring that an effective communication pathway exists.
  • To provide a holistic person-centered approach to care that meets the specific needs of individuals by enabling an increase in their independence, improving health outcomes, and reducing hospital admissions or long-term care.
  • To initiate advance care planning discussions and complete RESPECT forms.

Detailed job description and main responsibilities
  • Provide team leadership on daily basis and be responsible for shift co-ordination on a rostered basis.
  • Be responsible for clinical supervision and appraisals for designated supervisees
  • Support learning within the team through teaching and competency sign off.

Person specification

Experience
Essential criteria
  • Varied nursing experiences in a variety of clinical settings
  • Evidence of a range of clinical competencies appropriate for the role
  • Experience with EOL patients and EOL care


Desirable criteria
  • Experience of supervising other staff
  • Experience of working with the ageing population
  • Experience of leading a team
  • Community Nursing experience

  • Access to a car


Desirable criteria
  • Has a full driving license and business insurance


Qualifications and/or Professional Registration
Essential criteria
  • Registered General Nurse Qualification
  • Mentorship/NB 998 or equivalent


Desirable criteria
  • Experience of working with the ageing population


Skills and Knowledge
Essential criteria
  • A clear understanding of the role of admis-sion avoidance and responsive care, includ-ing Home first
  • Promote independence and choice in the undertaking of patient care
  • IT literate or willing to achieve within an agreed period
  • Broad range of nursing skills including cathe-ter care, medication management, wound care, IV drips, venepuncture and injections
  • Ability to manage the deteriorating patient and escalate as require through appropriate clinical pathways

  • Ability to challenge the decisions of others
  • Ability to manage complex situations
