Job overview

We have an exciting opportunity that has arisen and are looking for a motivated and experienced nurse or therapist, with the relevant knowledge and commitment to provide excellent clinical expertise in discharge planning, who has also demonstrated competence and experience in leadership and management to apply for the post of Team Lead within the Hospital Rapid Discharge Team, (HRDT) based at the Princess Royal Hospital.

The role will be crucial to providing clinical expertise and operational leadership, ensuring effective governance for the staff working in the team.

The role will be to provide clinical leadership to the team, supporting with clinical decision making to ensure patient flow from the acute trust through to the community services

You will work closely with the multidisciplinary team do deliver services and a provide a positive patient experience with regards to discharge planning.

You will need to develop and build on key partnerships, working to ensure effective whole system working, through effective communication and networking with other disciplines and by working across professional boundaries

Main duties of the job

To provide Operational Leadership, ensuring effective governance for all staff working in allocated area or service.

To be accountable for the direct delivery of services within a defined clinical area.

To work closely with multi-disciplinary leads to ensure the delivery of high quality multi-disciplinary care and positive patient experience.

To work with multi-disciplinary leads to deliver services that support skill sharing across professions and bandings and develops a competency driven, flexible and responsive workforce.

To report directly to Clinical Service Manager on team performance, operational implementation of work streams and service delivery issues.

To manage a delegated budget within service area.

To perform line management duties in line with Sussex Community NHS Foundation Trust policies and procedures.

To embed the principles of interdisciplinary working and cross skilling for all staff, including implementing competency frameworks.

To lead on audit and participate in research as appropriate, to inform future service development.

To develop key partnerships, working to ensure effective whole system working, through effective communication and networking with other disciplines and by working across professional boundaries.

To deputise in absence of Clinical Services Manager

Detailed job description and main responsibilities
To plan and allocate workload across area of responsibility, to ensure sufficient resources are available to deliver patient care.

To ensure effective lines of accountability are in place for all bands of clinical staff.

To lead communication pathways within the service through team meetings and ensure an effective governance structure is in place.

To ensure effective communication systems within defined area of responsibility to ensure staff feel informed and involved at all times.

To report directly to Clinical Service Manager on team performance, operational implementation of work streams and service delivery issues.

To work collaboratively with other leads to facilitate effective operational management of allocated services.

To work collaboratively with SCFT service managers, professional leads and clinicians and those from partnership organisations and across local health economy.

To represent the teams at wider SMTs, leadership exchange events, transformation meetings and any others appropriate.

To ensure services continue to develop in line with Trust transformation programmes and the SCFT Clinical Strategy

To line manage allocated staff to include, recruitment process, sickness and absence monitoring and disciplinary/ performance issues.

To report, investigate and manage incidents, sharing learning from incidents and risk with the team and wider organisation as appropriate.

To manage complaints in accordance with Trust policy and share learning across the team.

To work with multi-disciplinary leads to deliver services that support skill sharing across professions and bandings and develops a competency driven, flexible and responsive workforce.

To ensure effective delegation as per Trust guidelines and to support the implementation of the Calderdale framework and skill sharing initiatives.

To ensure all staff attend statutory and mandatory training.

To provide performance information to service managers to support the delivery of KPIs, management of demand and capacity and to inform future workforce, skill mix requirements and service development.

To manage service delivery for daily operations, seeking support and guidance from clinical service manager as required.

To monitor demand and capacity within designated area, to ensure sufficient resources are available to maintain service delivery.

To work with clinical leads to monitor performance, collect and analyse data, develop practice and standards of care.

To manage personal and budgetary processes (including; recruitment, EPRF, expenses approval, absence management, process orders).

To be responsible for ensuring effective induction for all staff within the area of responsibility and relevant students.

To maintain own professional competence to practice.

To ensure that all activity is person centred and MDT focussed.

To use advanced skills and expert knowledge to ensure that care is based on best available evidence to improve health outcomes and promote health and wellbeing of patients.

To act as professional role model for designated service area and uphold the Trust Values at all times.

Qualifications
Essential criteria
  • Degree / postgraduate Diploma / or equivalent HCPC / NMC Registration
  • Management qualification or willingness to undertake
  • Evidence of a range of Clinical competencies appropriate for the role
  • Evidence of Continued Professional Development
  • Evidence of leadership education / development or a willingness to undertake this
  • Significant experience of managing a team or other relevant management experience
  • Significant experience of supervision provision
  • Significant experience of working in the clinical setting
  • Experience of managing a multi-professional team / collaborative working

  • Post Registration qualification / training to masters level or equivalent postgraduate training and experience
