Job overview

As a Senior Case Manager, the post holder will work as part of the Provider Collaborative team in delivering an effective service to ensure safe and effective care pathways are in place for all service users within Kent and Sussex in specialised learning disability, autism and mental health care settings within the NHS and Independent sector.

To lead and manage the cases of people referred to and/or in tier 4 CAMHS services, ensuring the values and principles of care and treatment for all young people are integrated into the work of the Provider Collaborative and wider system partners.

To provide clinical support to the Provider Collaborative in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.

The role supports the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.

Main duties of the job

Provision of highly skilled clinical expertise to the Kent and Sussex Provider Collaborative and system partners and individual professionals and team across the local networks.

Ensuring effective care pathways, within and across Provider Collaborative Boundaries.

Resolution of complex or disrupted care pathways in conjunction with colleagues across England (and within UK for service users placed in services outside England). Support to local inpatient services in the admission and discharge planning of complex young people including those requiring transfer or repatriation.

Supporting the Provider Collaborative team in ensuring that services within the footprint are compliant to all specifications and guidance.

Ensuring the delivery of high-quality services that are value for money

Ensuring case management systems and record keeping are maintained and kept up to date

Hold a caseload of complex cases across two counties and act as a point of support and escalation for other case managers in managing complex cases

Detailed job description and main responsibilities
Improving quality and outcomes
  • To provide clinical assurance of the quality of the care provided by specialised services within the Provider Collaborative
  • Supporting the team in the assurance of specialised services compliance to applicable specification and guidance.
  • The post-holder will support the links with the Integrated care system and their commissioned services within the PC area to ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages.
  • Managing individual patient pathways focusing on those with complex presentations to ensure high quality services and care planning outcomes are achieved
  • To support and facilitate professional networks in considering options for young people as alternatives to admission and to expedite discharge. To be able to influence network decision making through sound clinical advice
  • To act in respect of Safeguarding Policies and Procedures as required

Enabling clinical leadership
  • To ensure that clinical leadership is central to the delivery of all of Kent and Sussex PC activities
  • To support and participate in clinical/professional supervision

Enabling patient and public involvement
  • To lead and direct case managers to act as a champion for patients and their interests and involvethe public and service users.
  • To ensure all public and patient contact is of the highest professional standard
  • To ensure that services have mechanisms in place for patient and carer involvement in their care plans
  • To ensure improvements in the pathway for Provider Collaborative patients accessing specialised services to improve patient experience and outcomes.

Promoting equality and reducing inequalities
  • To uphold organisational policies and principles on the promotion of equality
  • To create an inclusive working environment where diversity is valued, everyone can contribute, and everyday action ensures we meet our duty to uphold and promote equality
  • Provide education and support for others involved in the care of patients from specialised settings, working to reduce stigma, misconception and alienation of patients and their families
  • To monitor and review equity of access for patients and agree improvement actions.
  • To champion the parity of esteem of children and young people with mental health needs in networks and local systems

Partnership and cross boundary working
  • To work across providers within the NHS and Independent Sector
  • To work with all other specialised commissioning teams and ICS's in the management of cases, specifically those with complex or disrupted care pathways.
  • To work in collaboration with ICS's and services within the footprint to provide the effective referral and aftercare components of the commissioned pathways.
  • To work in collaboration with local authorities, third sector and other statutory bodies in the management of effective usage of the specialised mental health services and in the planning for discharge from specialist units.
  • To support the development of effective discharge pathways for those with complex multi-faceted needs.

Leadership for transformational change
  • To deliver a collaborative and influencing style of working, negotiating with others to achieve the best outcomes.
  • Work with key stakeholders, patients and their families to achieve national standards.

Using insight and evidence for improvement
  • To ensure that safe and effective care pathways are in place to facilitate the appropriate pathway for the patient through specialised services, including the identification and reporting of deficits within service provision
  • To ensure the appropriateness of the placement for the service user.
  • Ensure case management systems and record keeping are maintained and kept up to date by case managers
  • To provider insight and intelligence regarding system blocks and issues that impact care pathways

Developing an excellent organisation
  • To actively support the development of individuals and the team through appraisal, personal development planning, coaching and mentoring
  • To work in partnership with others and as part of cross directorate teams to deliver successful outcomes
  • To support the organisation's ways of working and model its values

Person specification

Qualifications
Essential criteria
  • Registered Mental Health/Learning Disability Nurse or other registered health or social care professional
  • Possess a relevant additional professional qualification e.g., masters level, or equivalent level of experience of working at a senior level in specialist area.

Knowledge & Experience
Essential criteria
  • Detailed knowledge of legal issues pertaining to the specialist service including MHA 1983, Children's Act, S117 after care, Transforming care, Safeguarding and child protection legislation
  • Detailed operational knowledge of specialised services, including risk assessment/management
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
