Job summary

We have an exciting opportunity for Senior Community Physiotherapist in our South Community Therapy Team to come and work in the service delivering the aim above via holistic, personalised, therapeutic care. We are currently offering roles in either of our Crawley, Horsham or Mid Sussex teams. You will work as part of well-established wider Multi-Disciplinary Teams (MDTs) across our local communities, where you will provide support across both proactive community rehabilitation and urgent care pathways. Our teams work flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm.

We will support you to make full use of your skills at whatever stage of your career to:
  • Provide specialist, holistic assessments, and reviews to improve or maintain quality of life.
  • Support individuals to take increased ownership and control over their health through rehabilitation interventions, personalised care planning and goal setting.
  • Provide an urgent response to minimise crisis situations annd the eed for emergency interventions or hospital admissions.
  • Improve access to existing services through a coordinated MDT approach.

Main duties of the job

In this role, you will use your skills to undertake assessments and plan treatment and interventions for patients with a variety of conditions, identifying ongoing needs and making appropriate onward referrals to our partners.

You will contribute to the co-ordination of the multidisciplinary team through the allocation of visits. You will also support the clinical and operational leads in the supervision and development of colleagues and will be supported to develop your clinical and non-clinical capabilities across the four pillars of practice.

You will need to able to drive with a full UK licence due to the geographic area covered by the team.
