Job summary We are looking for a highly motivated and enthusiastic Physical Health Support Worker to join us as a Band 3 Access Facilitator. You may already be qualified to NVQ Level 4 Diploma in Health and Social Care, or equivalent qualification and holding a Care Certification would be of benefit. You will proactively engage clients with a history of severe mental illness that are not engaging with their GP surgery. The work includes contacting and building relationships with clients (typically by telephone), encouraging clients to attend surgery appointments face-to-face, and undertaking physical health checks in various locations. As part of SPFT's Community Transformation , you will develop constructive and supportive working relationships with your allocated PCN and GP Practices. you will also work effectively and positively with SPFT's other community mental health services, and voluntary agencies. Main duties of the job You will work flexibly across Brighton and Hove as a vital member of the Mental Health Multidisciplinary Team to ensure a holistic, joined up approach for patients. You will be required to:
  • undertake outreach with hard-to-reach clients (including to non-clinical settings), to deliver comprehensive physical health assessments
  • work with clinicians and attend meetings across primary and secondary care to ensure individuals are referred to appropriate follow up interventions
  • work with individuals to develop physical health care goals and plans to promote and encourage self-management of physical health
  • Support the people you work with to develop relationships with key organisations offering health improvement interventions
