Job summary Are you a nurse with a passion for improving care for service users? Does a role focussed on reducing 'Out of Area Placements' and improving internal bed flow appeal to you?

Our 'Multi-disciplinary In Reach Support Team' (MDIST) is seeking a Band 7 join us as a Senior Mental Health Practitioner.

In this role you will work collaboratively with the multidisciplinary team and within Adult and Older Adult Services, to help us provide this provide this opportunity for the people of Sussex.

Working both remotely and travelling around Sussex, you will join our team as we set out to work collaboratively within the multidisciplinary team and with Adult and Older Adult services to deliver service aims.

Main duties of the job As a Senior Mental Health Practitioner in the new MDIST you will be responsible for the review of patients placed in beds outside of Trust stock. Working with Assessment & Treatment services, Crisis Resolution & Home Treatment and specialist placement teams, you will offer advice, consultation and input to a range of inpatient providers and community teams to support successful timely discharge. You will be expected to provide specialist interventions for referred clients, to promote and develop the role within the clinical area and to act as lead specialist within the MDIST. You will be pivotal to identification and resolution of barriers to discharge, including working with patients' local teams to ensure seamless transfer. You will play a crucial role in developing this service and will help us to support acute inpatient and community services to work closely together, to promote and ensure that all service users are given the best continuity of care and treatment.
